CarePossible, Inc

The mission of CarePossible is to make addiction treatment and mental health care possible for veterans and low income families.
 
There are four corners to CarePossible:
 
1. Addiction treatment--we partner with treatment centers to provide care to addicts who are ready for healthy change.
 
2. Mental Health Care--we partner with psychiatrists, psychologists, therapists, counselors and life coaches to provide free mental health care to those seeking help but unable to afford it. We also provide free confidential care for members of our military (active duty and vets) and their families.
 
3. Community Education and Awareness--we actively lead or participate in community events to spread awareness about mental health care and addiction issues to help fight stigma as well as prevent suicide and overdose--both of which are currently at an all time high. 
 
4. Military and Their Families--Currently there are over 22 military suicides each day, as well as over 300,000 Iraq/Afghan vets with PTSD. CarePossible provides free, confidential care and counseling to our military and their family members to save lives and save families.

Centennial Farm Foundation

The Mission of the Centennial Farm is to raise support for the programs of the Centennial Farm. The Centennial Farm has accepted the challenge of bringing agriculture education to the public, with a special emphasis on children, who will be tomorrow's decision makers and consumers. The Centennial Farm serves guests with a variety of programs. Most of the participants come from schools in Orange County however it is a unique facility that attracts visitors from surrounding counties. The Farm also welcomes guests on self-guided tours. Agriculture is a $23 billion industry in the State of California, making it the leading agricultural state in the United States. And yet most children are not aware of the importance of farming and how food gets to their plate. They also learn that agriculture employs 1 in 10 of all employees in California and could be a viable and stable option for employment. Additionally, in 2009, 26% of the students tested for the Fitnessgram in Orange County were considered to have unhealthy body weight. With childhood obesity at this rate, learning about food and healthy choices is more critical than ever. While the Orange County region has a reputation as an affluent community, many of the children are from low-income and at-risk families. According to the OC Community Indicators, only 53% of families in Orange County can afford to buy a home and to rent a 1-bedroom apartment a household earning minimum wage would need to work 131 hours per week. Therefore in most homes both parents are working, multiple jobs in many cases. The Centennial Farm offers an after school program as a safe, enriching option during the critical hours of 3pm to 6pm. In addition, many of our local schools have cut back or eliminated field trips. Because the Centennial Farm is unique, educational, and free with available scholarships for transportation, it is many classes only field trip of the year.

Clinic in the Park

 Connect •Screen •Educate. Using public space, Clinic in the Park is a health collaborative that offers free one-stop-shop services to: 1) connect underserved children to health services, public benefits, and community resources; 2) provide safety net screenings, including screenings for oral health, vision, hearing, medical and dental home; and 3) deliver health education with safety equipment to keep kids safe such as car seats and bicycle helmets. Community Health Initiative of Orange County (CHIOC)

The mission of the Community Health Initiative of Orange County (CHI OC) is to connect all children, families, and other adults to affordable and high-quality healthcare and social services. Reaching more than 20,000 residents in 2017, we specialize in providing access to healthcare for racial and ethnic minority populations and those that are economically and environmentally disadvantaged. 

Our Community Health Access Program helps Orange County residents enroll into healthcare coverage; navigate                health and social services systems; and utilize and maintain their coverage. In addition, CHI OC has a long history           of reducing fragmentation and improving Orange County’s healthcare system through our Regional Health Leadership initiatives. As a training hub for healthcare enrollment agencies in our region, we convene the OC Health Access and Enrollment Task Force, comprised of 248 members from 49 agencies. Goin Native Therapeutic Gardens

Goin Native Therapeutic Garden’s mission is to utilize public and private spaces to provide horticultural education, emotional & physical rehabilitation, and practical life skills to people of all ages, abilities and backgrounds. Nature is the healer. We plant with purpose in nature connecting people with the environment and each other. We gather new friends each week at the Los Rios Park gardens as we tend to the certified butterfly gardens as a whole. Our collaborative work has expanded with (CUSD) the ATP program with additional community partnership. We currently work with OC Bridges adult day program, creating a work type environment where clients sign in and out and have a task schedule. The weekly garden visits empowers each developmental disabled adult, giving them a purpose and meaning for their work while supporting our park goals.We have been adopted by the philanthrophy group, National Charity League, where moms and daughters volunteer during school breaks working side by side in the garden. Our all ages, background and abilities motto serves us well as we connect in the garden with the general population working as a team focusing on the tasks at hand and not our differences. Nature is an equalizer, it gives us life, we take from it and it takes from us. We are emotionally healthier when we connect in nature. Our goal is that the Los Rios Park Garden Angel Program concept will become a model for other cities to adopt to connect community to care for the local park gardens while the gardens care and nurture the people that tend to them. We live in a time of personal isolation with addiction, anxiety and depression on the rise. The garden programs we've created connect people, giving them a plan and purpose for their lives, halting the need to isolate. Our big idea is to create more park opportunities in our city and surrounding cities for people to tend and heal their emotional and physical selves while increasing the pollinators that help the gardens.

The G.R.E.E.N. Foundation mission statement is Gathering Resources to Educate and Empower through Networks. This is accomplished through collaboration, promoting the importance of early detection of chronic health issues that highly impact African Americans (i.e. cancers, heart disease, diabetes, stroke, nutrition, and healthy living through daily decisions and spirituality).

The purpose of the organization to improve the overall health status of African American communities, to build an inclusive, caring community of healthy women and families. African Americans have the highest incidence rates in heart disease, cancer, stroke, diabetes, hypertension and kidney diseases. (CDC, Office of Minority Health and Health Disparities -February 10, 2014). According, American Cancer Society to reduce mortality rates early detection through screenings play a key role along with empowering people to make healthier daily decisions.

Laguna Beach Community Clinic

The mission of the Laguna Beach Community Clinic is to provide excellent medical care regardless of the patient’s ability to pay.

The patients we serve are at risk of foregoing early medical care of conditions, which would negatively affect their morbidity rates and also burden the healthcare system in the long run. Their needs to access healthcare include finding:

· A registered Medi-Cal provider. Families enrolled under the Affordable Care Act are challenged to find a provider that accepts Medi-Cal.

· A provider which accepts all uninsured patients without restriction. Those who don’t qualify for government subsidized insurance or choose to opt-out may be turned away from medical providers that cap the number of uninsured patients or visits. 55% of LBCC patients do not have health insurance.

· Care that is accessible linguistically and geographically.

· High quality care.

LBCC meets each of the patient needs discussed above. We accept families with Medi-Cal health insurance. The clinic also accepts patients without insurance. Patient payments are based on a sliding fee scale and ability to pay; for those who cannot pay, LBCC covers the entire visit cost. Inviting patients to pay what they can afford preserves dignity; affordable care helps patients access care consistently. We have several providers who are bi-lingual and speak Spanish fluently. LBCC is the only clinic in south Orange County to offer the full range of primary care, weekend hours, and walk-in availability.

These health risks are lifelong, increase with age, and can result in early death. This is why we’re kickstarting our own wellness revolution, one specifically for kids and families fighting cancer and the lifelong effects of cancer and related life-threatening treatments.

Inspired by SuperMax Wilford, a brain cancer fighting superhero, MaxLove Project is a volunteer-powered, grassroots nonprofit organization founded to help SuperKids thrive against cancer and life-threatening conditions. A Christian response to homelessness.
OFT walks alongside our neighbors experiencing chronic homelessness, connecting them to agencies with the goal of ending their cycle of homelessness, ensuring their journey back to RESTORATION.
OFT is the light at the end of the tunnel!  Our Father's Table, OFT,  has a solution that breaks the cycle of chronic homelessness, 1 person at a time. Our work is important because it addresses homelessness at its cause knowing chronic homelessness is a symptom of a deeper-rooted problem.  OFT changes lives, by giving a hand up, not a hand out! Our work is cost effective & it does not create another program.  Our unique approach puts us in the center of their life as mentor, advocate & most importantly genuine friend giving unconditional friendship & guidance.  
 
The crisis of homelessness in OC has been front & center as the media exposed the day-to-day tragedies unfolding in the homeless community.  How can we forget the horrible story that was reported in March of 2018 when a family of 4 died in their mini-van that was also their home.  Then there is the heartbreak of our veterans, who make up 10% of our homeless population. These scenarios could have been avoided if we had more trained Care Navigators so we could expand our canvas area outside of the southern beach cities of OC. 
 
The numbers speak for themselves... In a 12-month period (2014-2015) in OC:
  • $299 million: Governmental & non-governmental entities spent on addressing homelessness. ($106 million spent on all types of housing for the homeless. $193 million spent on healthcare, criminal justice system, governmental & non-governmental agencies.)
  • $100,759: Average annual cost for 1 chronically homeless living on the street or in emergency shelters (OC United Way Cost Study on Homelessness, 2016)
But there is a successful & Christian solution! $12,385 is the cost for OFT to lift 1 chronically homeless person off the streets; a savings of $88,374 per person. OFT averages 30 lifted each  year, a savings of $2,651,220 per year.

RTH Stroke Foundation

The mission of the RTH Stroke Foundation is to eliminate stroke whenever and wherever possible. People suffer 795,000 strokes in the U.S. every year, 144,000 of which result in death.  Stroke is the fifth leading cause of death, and although most strokes occur to people 65 or older, stroke can attack people of any age or gender.

But experts in vascular medicine say that fully eight out of every ten strokes can be prevented. That means we have it in our power to reduce the annual incidence of stroke from 795,000 to less than 160,000, The key is awareness. Only a fraction of the American population understands the lifestyle and medical steps they need to take to keep them out of the high-risk stroke pool. Fewer than one in five can recognize the symptoms of a stroke at its onset.

The RTH Stroke Foundation aggressively pursues this under informed segment of our population. Every year we hold free stroke awareness seminars for several thousand attendees at which experts in the field explain what people have to do to reduce the likelihood of stroke. We conduct free diagnostic screenings for aneurysms, blood pressure, carotid narrowing and cholesterol. These screenings identify people at various stages of risk, including some whose lives may have been literally saved because they had no idea that they had an aneurysm or a severely restricted carotid. And we hold free monthly stroke support meetings to help stroke victims, their families and caregivers learn how to adjust to the stroke victim’s new normal.

The quest to reduce the incidence of stroke so drastically may seem Quixotic to some, but not to us. It’s the reason we come to work every day.

Strength In Support (SIS)

How soon do you want to come in?"

75% of funds raised locally stay in our community to help Orange County women, families and men. Womens Transitional Living Center, Inc.

WTLC helps individuals and families escape the depths of domestic violence and exploitation.

Since 1976, our guiding principal has been to end the cycle of domestic violence and exploitation by providing comprehensive services grounded in evidence-based practices. Through a strengths-based, survivor-driven and trauma -informed holistic approach, the survivor’s perspective of their priorities establishes the plan to create a healthier and safer life.

WTLC’s programs are designed to be flexible and responsive to each survivor’s unique needs—each participant builds their recovery plan with their own priorities in mind. In this way, survivors from a range of backgrounds and experiences are able to receive tailored support that addresses their personal barriers to long-term stability. Survivors of domestic violence and human trafficking, as well as their children, receive services designed to meet their individual needs and adapt as necessary throughout their time with WTLC.

WTLC accepts populations that would normally struggle to find support including re-entry survivors coming out of the prison system, survivors with ongoing mental illness problems, survivors struggling with alcohol and drug addition, transgender survivors, emancipated youth and survivors with animal companions.

Last year, WTLC served over 9,000 children and adults through our housing, supportive services and prevention programs. These achievements were made possible by the generous support WTLC receives from individuals, families, corporations, foundations and government agencies.

Domestic violence and human trafficking are major public health crises, and community support and engagement is integral in WTLC’s work to create a world without violence.

Young Lives Redeemed takes transitional-age emancipated foster youth from homelessness to stability and self-sufficiency through engagement in our Wellness Program, which includes connection to trauma-informed mental healthcare, substance abuse treatment, sustainable housing, secondary and post-secondary education, vocational programs, job training and employment. We envision a world where no former foster youth are living on the streets or incarcerated. We seek to change lives and create hope.
