Background Statement
Founded in 2018, Celebrating Life Community Health Center (CLCHC) honors the legacy of Kristina Sergeyeva, who tragically passed away at the age of 24. Her father, Sergey Sergeyev, was inspired to create a healthcare organization dedicated to serving vulnerable communities after experiencing firsthand the challenges of being a refugee and enduring the sudden loss of his daughter. In this mission, Mr. Sergeyev partnered with Sylvia Caliendo, the wife of Dr. Caliendo, who passed away in June 2019. Dr. Caliendo had operated a private practice in Mission Viejo since 1994, dedicated to providing care for medically underserved and uninsured children in Orange County. Both Kristina Sergeyeva and Dr. Caliendo shared a deep commitment to advocating for those most in need. CLCHC is the realization of the Sergeyev and Caliendo families’ shared dedication to continuing their loved ones’ passion for accessible, high-quality healthcare for Orange County’s most at-risk and underserved populations.

Through community outreach and a comprehensive range of services, we strive to uplift and empower every individual who walks through our doors—fostering a community where health and dignity are accessible to all.
Needs Statement
Did You Know? Heart disease is the leading cause of death. Suicide is the second-leading cause of death among youth ages 14 to 25. Fentanyl-related deaths surged by 321% between 2016 and 2022. These alarming trends highlight the urgent need for accessible, high-quality healthcare. At CLCHC, we are committed to addressing these challenges through specialized programs in behavioral health, heart screenings, and pediatric, youth, and college care. Additionally, neonatal care, obesity, cancer, cardiovascular disease, diabetes, and respiratory disorders remain major health concerns across Orange County. The rise in these conditions is a symptom of a larger issue: lack of access to comprehensive healthcare and support. We are dedicated to bridging this gap and ensuring that every individual receives the care they need to lead a healthier life.

Our long-term vision is to expand into the most underserved and neglected areas, ensuring that no one is left behind. In the short term, we are focused on strengthening our newly acquired clinics in Costa Mesa, Santa Ana, and Irvine. These facilities require urgent improvements, both in infrastructure and staffing. We need more specialists in mental health, pediatrics, and neonatal care to address the growing healthcare needs in our community. Recent data highlights critical gaps in student mental health and heart disease treatment—gaps that lead to preventable disabilities and deaths. Addressing these issues requires proactive, preventative, and actionable solutions. We invite you to join us in this vision. Statement from the Board Chair/President
‘HOPE PLEDGE’ Behavioral Health - Heart Screening - Primary Care Outreach
A) Behavioral Heath - CLCHC is focused on access to behavioral health and pediatric care. Adolescent mental health continues to worsen. In 2021 more than 1 in 5 students seriously struggled with anxiety, PTSD, suicidal ideation, and other health concerns. Greater than 25% of people in the United States are experiencing some form of it. That’s approximately 50 million people and 55% have not received any treatment. Good mental health helps individuals develop resilience to cope with life events and greater socio and economic stability. B) Heart Health - screen, detect, and manage structural and cardiovascular conditions, and preventative educational heart health programs. It is a threefold tactical plan of heart screening for newborns through young adults and adult wellness exams; and behavioral health assessment that evaluates individuals at risk with heart disease. The first concern is childhood health. Socioeconomically disadvantaged children have greater exposure to cardiovascular risk factors and undetected structural heart disease. This status affects levels and trajectories of life expectancy, income, and wealth during adult years. The second concern is mortality rates of Asian, Black, and Hispanics populations post covid-19. In underserved groups there is a disproportionate rate of individuals that lacked access to structural and cardiovascular heart screening by licensed clinicians. Cardiovascular related deaths jumped from 874,613 in 2019 to 928,741 in 2020, the largest year increase since 2015, and greater than 50% of the US population has some type of heart disease. Not only is this a health crisis. It is an economic crisis that costs the US about $219 billion yearly with no end in sight (01/25/2023 American Heart Association Journal). Thirdly, a large and growing body of research shows that mental health is associated with risk factors of heart disease. Mental health research studies of bi-polar, anxiety, posttraumatic stress and chronic stress disorders are associated with risk factors for heart disease that developed before a diagnosis or during treatment. The extended period associated with mental health illnesses may increase cardiac reactivity of increased heart rate, reduced blood flow to the heart and brain, and heighten levels of cortisol. Specific population including veterans, women and ethnic minorities are at greatest risk (May 6, 2020, Heart Disease cdc.org). The practice of identifying individuals with heart disease or at-risk individuals will be an aspect of behavioral health screenings. C.) Primary Care Outreach - Community expansion of health care as a primary medical practice expanding into the most vulnerable neighborhoods, Costa Mesa, Santa Anna and Irvine. it is possible to transform the healthcare system to rein in costs, improve quality, and expand access. CLCHC’s long-range business goal is expansion into the most vulnerable and neglected communities. The short-term goal is recent acquisitions of medical clinics in Costa Mesa, Santa Ana, and Irvine. Immediate operational need is to improve these neglected deteriorating buildings; and hire more doctors with specific specialties in mental health, pediatric and neonatal care. The most recent data shows there’s a student mental health crisis; and a heart health treatment gap, resulting in needless death and disabilities. Closing these gaps in mental health care and heart health takes appropriate, preventative, actionable processes. Please join us in the vison to manage these treatment gaps. CEO/Executive Director
Executive Summary Founder and CEO Sergey Sergeyev brings extensive experience in the medical field, including his tenure at Share Our Selves Medical Clinic (SOS), where he successfully increased patient volume and improved profitability. His career took a profound turn following the tragic loss of his college-bound daughter, Kristina, to an undiagnosed heart condition. This devastating event became the catalyst for his life’s mission—to provide accessible, high-quality healthcare to those in need throughout Orange County. While a corporate-level position had its merits, Sergey’s vision shifted toward community-based, hands-on care, ensuring that healthcare reaches those who need it most. Under his leadership, Celebrating Life Community Health Center (CLCHC) is committed to bringing care directly to underserved communities. The organization is actively collaborating with Someone Cares Soup Kitchen in Costa Mesa and is in discussions with Family Assistance Ministries in San Clemente and Families Forward in Irvine to develop a shared services model that maximizes impact. CLCHC is an evidence-based, nonprofit family practice, dedicated to breaking barriers to care and improving health outcomes for Orange County’s most vulnerable populations.
CEO/Executive Director Experience
Sergey Sergeyev, MPA, CHC, CHPC, has served as Celebrating Life Community Health Center’s (CLCHC) Chief Executive since its founding in December 2018. Under Sergey’s leadership, CLCHC has transitioned from a private family practice to a licensed community health center, implementing the clinical model of care required for a successful and sustainable health center program. Mr. Sergeyev established the community-based board of directors and integrated comprehensive healthcare services across the lifespan, including medical, dental, behavioral health, and social services to underserved and uninsured communities of Orange County. Sergey brings over two decades of continuous executive management experience in managing community health centers and Federally Qualified Health Centers (FQHC). Immediately before founding CLCHC, Sergey was the Chief Financial and Privacy Officer at Share Our Selves (SOS), a mid-sized FQHC in Orange County, wherein he successfully managed SOS from a $4M budget to a $22M organization, including expansion to six clinic sites and two administrative locations. He was also responsible for creating in-house financial management, accounting, and billing departments. Prior to Share Our Selves, Sergey served as the Interim CEO at the To Help Everyone (THE) Clinic, a well-regarded FQHC in Los Angeles, and as Chief Operations and Financial Officer (COFO) at Saban Community Clinic, one of the largest FQHCs in Los Angeles County. With his firsthand knowledge of FQHC clinics’ financial management systems, Sergey brings a wealth of experiences as a nonprofit senior executive to his role at CLCHC. Sergey holds a Bachelor of Arts in Business Administration (BA) with a double emphasis in Accounting and Finance from Fresno Pacific University, a Master of Public Administration (MPA) in Healthcare Management from Keller Graduate School of Management in Chicago and is pursuing his Ph.D. in Organizational Psychology from Grand Canyon University, as he enters the dissertation phase. Sergey also holds certifications in Healthcare Compliance and Healthcare Privacy Compliance. Further holding personal membership in the Healthcare Compliance Association, Healthcare Financial Management Association, Institute of Management Accountants, and National Association of Community Health Centers. Sergey is fluent in English, Russian, and Ukrainian.

Our Story
CLCHC was founded in December 2018, preserving the legacy of Kristina Sergeyeva who passed unexpectedly in October 2018 at just age 24. Mr. Sergeyev joined forces with Sylvia Caliendo, the wife of Dr. Caliendo who passed away in June 2019 and operated a private practice in Mission Viejo, established in 1994, dedicated to serving the low-income and medically uninsured and underinsured children residing in South Orange County. Kristina, who passed away at just 24 years old, was just beginning her journey in community health having worked at a local community health center and had recently graduated from UC San Diego with a Bachelors of Science in Biochemistry and Cell Biology. Ms. Sergeyeva was also enrolled in a master’s degree program at the time of her death. Mutually, Dr. Caliendo and Ms. Sergeyeva were committed to serving and advocating for the low-income and underserved individuals residing in Orange County. CLCHC is result of the Caliendo and Sergeyev Families commitment to keeping their loved one’s passion and commitment to serving the most at-risk and underserved in South Orange County.
