Overview

Mission Statement

Our mission, in partnership with families and the community, promotes an environment of health, dignity and well being for seniors, disabled adults and caregivers.

Our commitment to the community is the driving force behind the Board's and Staff's dedication to follow our mission and offer innovative person-centered programs and supportive services that our families need.

Background Statement

For more than twenty years, Irvine Adult Day Health Services has been a leader in the industry and the only licensed adult day services provider in Irvine that addresses the specialized care our participants need. Our organization was established in 1994 by a visionary group of leaders who were dedicated to helping our most vulnerable disabled adults, seniors and their caregivers.

We offer an array of community based services and activities in a safe and welcoming environment lead by dedicated and caring professionals and volunteers.

The Center is licensed by the California Department of Public Health, and certified by the California Department of Aging. Programs offered: Adult Day Health Care (ADHC), Community Based Adult Services (CBAS), and Social Adult Day Program (ADP).

Impact Statement

Mind, Wellness, and Body. Irvine Adult Day Health Services’ adult day services program is a vital care option offered to adults and seniors managing Alzheimer’s disease, Heart Disease, and other chronic health conditions. Through early detection, assessment, and daily health monitoring and interventions, we help families stay together by reducing hospital visits and delaying nursing home placement.

The ability to access Adult Day Health Services can bridge the gap between a primary caregiver’s ability to sustain their responsibilities while ensuring the senior receives needed health services, socialization, professional interventions and supervision.

Our Center has embraced a ‘Person Centered’ care philosophy. This philosophy believes that healthcare is a partnership among care providers, individuals and their families. It encompasses compassion, empathy, respect and supporting the individual. We are enthusiastic to offer a program reflective of this philosophy. From care planning to activity programming, the nature of services have been individualized to meet the needs and interests of those attending our Center.

July 2017 marked the Center’s 23rd Anniversary. Since our opening, we have provided nearly 200,000 days of care to 1,117 adults and seniors. The number of lives we have touched exponentially increases with the support and resources provided to our caregivers. We are proud of our successes over the years and thrilled to have impacted so many families in the community.

On a daily basis, we see how our staff have touched our families' lives. It’s the little things that add up: seeing our participants faces light up during Pet Therapy visits, watching participants paint in our Memories in the Making® art program, and hearing expressions of relief and appreciation from caregivers.

We gauge impact from feedback provided through our annual satisfaction survey process. Notable quotes include:

·         “The Center provides peace of mind and respite to our caregiver and family members. There is peace of mind knowing that our loved one is in good hands of professionals.”

·         “My mother really enjoys going to the Center. She has reports that the staff are very nice to her and that she has made friends with the other participants. She looks forward to her time there.”

Needs Statement

Our team works in partnership with families to identify positive long term care solutions that keep families together. We support adults and seniors with chronic health conditions that can affect their ability to manage their basic needs. Our welcoming and dedicated staff, diverse programming and person centered approach makes us stand out as a valuable resource and expert provider in Orange County.

Approximately 85 percent of older adults have at least one chronic health condition, and 60 percent have at least two chronic conditions, according to the Centers for Disease Control and Prevention. For many older people, coping with multiple chronic conditions is a real challenge (National Institute on Aging 2016).

The ability to access adult day health services can bridge the gap between a primary caregiver’s ability to sustain their responsibilities while ensuring their loved one receives needed health services, socialization, therapies and protective supervision during the day. Our program provides this access and gives families options.

IADHS’ most pressing challenges include:

1) The need to increase community education on the benefits of adult day services, with limited funding and staffing resources.

2) We have observed a viable trend that our participants are enrolling at a higher acuity level, requiring increased therapeutic interventions and supervision needed to manage their basic needs, and physical, psychological and cognitive functioning.

3) The need to bridge the gap between fees collected and the actual cost of care (gap ranges from $32 to $51 per participant per day of care).

Program Long-Term Success

IADHS is the only provider in Irvine that addresses the specialized medical care our participants need. Adult day services is a cost-effective way to provide community based long-term care support and services for seniors and disabled adults with memory loss and other chronic health conditions.

We offer innovative health care solutions that are bundled together in one comprehensive day of care. Without our program, families would not be able to coordinate all of their care needs as effectively and comprehensively as they can at IADHS. Many care needs would go unsupervised or undetected without the daily medical supervision and care provided by our experienced team of healthcare professionals.

Program Goals:

·  Help maintain participants’ health & functioning while supporting their unique goals and personal interests.

·  Help participants increase their strength & balance to prevent falls

·  Give caregivers relief from providing 24/7 care

·  Help prevent emergency room visits & delay undesired nursing home placement

Program Success Monitored By  IADHS measures its success directly from feedback given by participants enrolled in our adult day services program and their caregivers. Feedback is shared regularly during program hours and is solicited annually through our Satisfaction Survey process. In a recent survey, caregivers shared they have confidence in the staff and feel relief that their loved one is in the program, safe and happy. With positive feedback from our families, we can see first hand the value of the care and services we provide. The smiles on our seniors’ faces tell us that we are making a difference in their quality of life.
Examples of Program Success

Annual Satisfaction Survey Quotes include:

  • “I feel that my health is better since I have been here and I feel more independent.” Participant

  •  “The biggest benefit is getting my husband out of the house and meeting people. His social skills were rapidly declining before he was coming to the Center.” Caregiver
  •  “The Center gives me peace of mind that my dad is being taken care of. I am able to work. He is happier and that makes me happy and less worried.” Caregiver

Collaborations

IADHS is a long-standing and active member of the Orange County Adult Day Services Coalition (Coalition), locally and the California Association for Adult Day Services (CAADS), statewide. Through the Coalition, we position adult day services as one united voice. With funding from the Irvine Health Foundation, we have developed new strategic priorities to increase awareness of adult day services through working with Influencers/Policy Makers, Caregiver Community, and Professionals in the industry.  A new Coalition strategy in the year to come is coordinating a training program for Coalition Member staff on Person Centered Care. With these associations, we have a broader reach for advocacy messaging, industry best practices, awareness and education.

IADHS partners with the OC Alzheimer’s Association in a variety of ways, through bi-monthly Caregiver Support Groups, advocacy, education, Memories in the Making®, and Alzheimer’s Walks. In the past year, our Lead Social Worker was invited to participate in a program to facilitate “Advanced Planning End of Life” sessions in the community.

Additionally, IADHS and the City of Irvine Senior Services work together to provide an invaluable community safety-net for vulnerable, at-risk seniors, including non-English speaking seniors. Through joint care management we are exploring ways to increase seniors’ access to medical and mental health care services to meet this growing community need.
